Return Goods Authorization (RGA) Request

Submit your repair orders online using our RGA (Return Goods Authorization) request form.

Here’s how it works:
- Complete the RGA Request Form. You will be asked to supply your contact information and repair product details.
- After submitting your request, you will be contacted by the
Customer Service Representative in your area. You will also
receive an E-mail with the Customer Service Representative's information.
- The Customer Service Representative will provide any pricing information. You will be asked to fax a purchase order in order to process your request.
- Once the purchase order is received, the Customer Service Representative will provide the RGA number. This number must be clearly marked on the outside of your return shipment.
- All returned goods must be shipped pre-paid. Shipping information will be provided by the Customer Service Representative.
